What kind of practice is Paradigm Health?

Paradigm is a longevity-focused concierge medical practice in Franklin, Tennessee, founded by physicians. Modern medicine is brilliant at treating disease and spends almost nothing preventing it — we take the other route. We measure your health in real depth, build you a personalized plan, and then stay with you and keep adjusting it.

Care is delivered by our own physicians, nurse practitioners, health coaches, and doctors of physical therapy, working from one shared plan.

Where are you, and do I need to come in person?

Our clinic is at 520 Duke Drive, Suite 200, Franklin, TN 37067, serving Franklin and greater Nashville. You can reach us at (615) 469-0703.

The assessment and the advanced testing happen in the clinic — DEXA, VO2 max, motion analysis and lab draws all need you there. Follow-ups and many everyday visits can be done by video or phone when that is the right call clinically.

What are the membership tiers, and what do they cost?

Life Plan is $5,500, paid once — the full longevity assessment and your written plan, with no ongoing commitment.

Life Coach is $8,250 a year — everything in Life Plan, plus ongoing monitoring, optimization visits, and medication management.

Life Team is $11,000 a year — everything in Life Coach, plus complete primary care. Paradigm becomes your PCP.

Life Navigator is $25,000 a year — everything in Life Team, plus complex-condition care, dedicated case management, home visits, and full white-glove coordination.

The annual plans are paid as $5,500 up front and then monthly for eleven months. A full side-by-side comparison is on the memberships page.

What does every membership include?

Every membership starts with the comprehensive longevity assessment: DEXA body composition, VO2 max and cardiopulmonary exercise testing, an advanced biomarker panel, motion and mobility analysis, and continuous glucose and metabolic insight — then a written, prioritized plan in plain language.

Annual memberships also include two comprehensive lab draws a year, a proactive physical therapy assessment, and 10 to 20 percent off infusions and lab packages in the clinic.

Does Paradigm become my primary care doctor?

With Life Team and Life Navigator, yes. Paradigm is the practice you call first — same-week sick visits in person or by video, prescriptions and refills, referrals and specialist coordination, and answers between visits instead of a trip to urgent care. You have a direct line to your physician and care team, with no phone tree.

Life Plan and Life Coach members keep their own primary care doctor. We work alongside them.

Do you take insurance?

Membership is paid directly to the practice and is not billed to insurance. It is not insurance and does not replace it — please keep your health plan for hospital care, specialists, emergencies, and your medications.

Your prescriptions are filled at your own pharmacy and are usually covered by your plan in the normal way, and outside laboratories and imaging partners bill under their own arrangements.

What happens at the first assessment?

It is a full-body, full-system look, far beyond a standard physical: body composition, cardiopulmonary fitness, a deep biomarker panel, how you move, your metabolic health, and a whole-person review of sleep, cognition, nutrition, and lifestyle.

Everything is then translated into one clear plan — written in plain language, prioritized, and built around your actual life. The how it works page walks through each step.

How do visits work once I am a member, and how do I reach my team?

You have direct access to your physician and care team. Call them, or message them securely through the member portal, whichever you prefer — you reach the people who already know you.

Rather than one annual physical, care is spread across the year: optimization visits, proactive check-ins, and a mid-year and year-end review of how your plan is working.

What testing do you do, and how often?

Your foundation year includes an extensive biomarker panel, DEXA, calcium CT and ultrasound, VO2 max and motion baselines, and a continuous glucose monitor, then screening and repeat testing through the year, with labs and DEXA repeated at the mid-year review and performance testing repeated around month seven.

Your physician personalizes which tests you have and when. We also arrange best-in-class screening through partners such as Cleerly, full-body MRI, calcium scoring, lung screening, and genomics.

We read your results against Paradigm optimal ranges, not just the lab's "normal" range — because not sick and thriving are not the same thing.

Do you write prescriptions and handle refills?

Yes. Medication and supplement management is part of every annual membership, and Life Team and Life Navigator include prescriptions as part of complete primary care.

Your physician prescribes electronically and sends the prescription straight to your pharmacy. Refills are handled by your care team — ask through the portal or by phone, rather than waiting until you run out.

Which pharmacy do you use?

Yours. Your physician chooses the medication on clinical grounds, and you choose the pharmacy it goes to — retail, mail order, or compounding.

Paradigm does not steer members to any pharmacy and has no ownership stake in, or financial relationship with, any pharmacy or drug manufacturer. If you ever want to change pharmacies, just tell your care team.

Do I have to be a member to buy labs, infusions, or a scan?

No. Lab bundles, IV infusions, DEXA scans, VO2 max testing, motion analysis, physical therapy visit bundles, and the medical weight-loss program are all available to anyone through our services and store page. Members simply pay less.

What is Calanthe?

Calanthe is our members-only health experience — the app and portal where you see your Paradigm Plan, your labs and device data explained in plain language, your trends toward your optimal targets, and where you message your care team.

Your whole record lives in one connected system, so every clinician who sees you has the full picture.

Can I add my family?

Yes — family members can be added to your membership. Ask us about family pricing and we will put together the right arrangement.

How do I cancel my membership?
