Paradigm Health does not provide emergency medical care and this website is not a way to reach us urgently. If you are having chest pain, difficulty breathing, signs of a stroke, severe bleeding, thoughts of harming yourself, or any other emergency, call 911 or go to the nearest emergency department. Do not use this website, a web form, email, or the member portal to report an emergency — they are not monitored around the clock.
Your medical care and membership are provided by Paradigm Health, PLLC, the licensed medical practice. This website and the administrative, scheduling, and billing services around your membership are operated by Paradigm Health Asset Management, LLC. Both are located at 520 Duke Drive, Suite 200, Franklin, Tennessee 37067. By using paradigmhealth.org, buying a service from us, or joining as a member, you agree to these terms. If you do not agree, please do not use the site.
Separate agreements may also apply to you — a membership agreement, a consent to treat, a financial policy, and our Notice of Privacy Practices. Where one of those signed agreements says something different from this page, the signed agreement controls.
Reading this website, filling in a contact form, or booking an introductory call does not create a physician-patient relationship. That relationship begins only when you become a patient of the practice and a Paradigm clinician accepts you into care.
Everything on this site — the philosophy, the descriptions of testing, the membership pages — is general information about what we do. It is not medical advice for you, and it is not a substitute for a visit. Never delay getting care, or ignore advice from your own clinician, because of something you read here.
What each membership includes is described on our Memberships page. Membership gives you access to the care and services listed for your tier. It is not insurance, it is not a health plan, and it does not replace one. All members and clients — on every plan and service — should still carry their own health insurance for hospital care, specialists, emergencies, and medications.
Some services shown on our Services & Store page are sold individually and do not require membership.
Prices are in U.S. dollars and are the prices in effect when you enrol. We may change our prices for a future term, and we will tell you at least 30 days before a change affects you. Card payments are processed securely by a third-party payment processor — your full card number is entered on their secure page and never touches our website.
Fees pay for access to the care and services in your tier. They are not payment for any individual test or treatment, and they are not billed to insurance. Some members may be able to apply HSA or FSA funds toward fees for qualified medical care; whether a fee qualifies depends on your plan and the tax rules, so please check with your plan administrator and tax advisor. Paradigm does not provide tax advice.
Term and renewal. Membership runs for a 12-month term and does not renew automatically — at the end of the year you decide whether to continue. The one exception: if you move into the ongoing Paradigm Member program after a year on the Life Team or Life Coach plan, membership then continues month to month at $500 per month until you tell us to stop. In that case we will remind you about 30 days before your 12-month mark so you can choose to opt out.
If a payment doesn't go through, we will let you know and try the card again — there is no late fee or penalty. If it still can't be collected, we may pause non-urgent scheduling (not your care) until it's resolved, and we'll work with you on a simple way to catch up or update your payment method.
You may cancel your membership at any time by telling us — either you or the practice can end the agreement, with or without a reason. If you have prepaid, we refund the prorated portion of your membership fee for the part of the term you did not use, less any costs already incurred. Any other refund of fees already paid is at the practice's discretion, in line with your signed membership agreement. When you leave, just let us know where to send your medical records.
Individually purchased services (for example an infusion, a lab bundle, a DEXA scan, or a PT package) are refundable before they are redeemed; once a service has been provided it is not refundable.
Your signed membership agreement is the controlling document. Where it says something more specific than this page, the signed agreement applies.
Please give us as much notice as you can if you need to change or cancel a visit — someone else is usually waiting for that time. There is no no-show or cancellation fee; we try to be as flexible as your life needs. Just let us know, and we'll find another time.
Some visits can be done by video or phone when your clinician judges that appropriate. A few things to know about telehealth:
Your physician decides whether a medicine is appropriate for you, and which one, based on clinical judgment and your health. Requesting a medicine — through the portal, at a visit, or anywhere else — is not a guarantee that it will be prescribed, and a clinician may decline if it would not be safe or appropriate.
You choose your pharmacy. We send your prescription electronically to the pharmacy you name, and we have no financial relationship with any pharmacy.
Controlled substances are prescribed only where clinically justified, in line with federal and Tennessee law, and may require an in-person visit, a treatment agreement, and a check of the Tennessee Controlled Substance Monitoring Database.
Keep your sign-in details private and do not share your account. Tell us promptly if you think someone else has access. You are responsible for what happens under your account. We may suspend an account if it is being used in a way that puts anyone's information at risk.
The text, images, design, logos, and the Paradigm and Calanthe names on this site belong to Paradigm Health or to the people we license them from. You are welcome to read, print, and share pages for your own personal use. Please do not copy, republish, or use our content commercially without our written permission.
Anything you send us through this site — a question, a comment, a suggestion — may be used to improve what we do, without obligation to you. Do not send confidential health information through a web form.
We sometimes link to companies we work with or think are useful. We do not control those sites and are not responsible for their content or their privacy practices.
We work hard to keep this site accurate and available, but we cannot promise it will always be correct, complete, current, or uninterrupted. To the fullest extent the law allows, the site is provided "as is" without warranties of any kind.
This section is about the website. It does not limit our responsibilities as your health care providers, and nothing on this page changes the standard of care we owe you as a patient.
To the fullest extent permitted by Tennessee law, Paradigm Health is not liable for indirect, incidental, or consequential damages arising from your use of this website.
These terms are governed by the laws of the State of Tennessee, without regard to its conflict-of-law rules. Any dispute will be brought in the state or federal courts located in Williamson County, Tennessee, and you and Paradigm agree to that venue.
We may update these terms. The current version is always posted here with its effective date. If a change is significant, we will make a reasonable effort to tell members directly. Using the site after a change means you accept the updated terms.
